After booking your appointment — either online or by phone — you will receive a confirmation email with a link to complete your intake forms. Please complete these as soon as possible to retain your appointment.
When arriving for your appointment, enter the Howard Med Arts building and follow the signage to our clinic. Suite 2 is the second door on the left when entering from the primary entrance.
You will be greeted by front desk staff who will check you in, confirm any insurance or billing details, and notify your clinician of your arrival. A waiting area is available for your convenience.
Your clinician will meet you in the waiting area and bring you to a private treatment room.
Following your session, front desk staff will assist with checkout and help schedule any follow-up appointments, if required.
Please bring any relevant imaging reports or have your physician fax them to us ahead of time so that we can incorporate that information into our assessment and treatment.
If you are comfortable, please bring or wear clothing that allows us to get to the treatment area if needed. Eg = shorts for a knee / hip assessment.
For gym sessions, please bring a water bottle to stay hydrated. We have a bottle refilling station in the gym for your use.
We are not an OHIP-eligible clinic and do not accept your health card to cover the cost of treatment.
We direct bill the majority of private insurance providers. Patients are responsible for any remaining balance at the time of service. As coverage varies by plan, we are not able to verify benefits in advance. Please contact your insurance provider if you require clarification.
Some providers do not allow direct billing or may require additional information from the patient. In these cases, you will receive a receipt for full payment, which you may submit to your insurance company for reimbursement.
For expedited care, please ask your physician to fax a referral and any relevant imaging to 519-258-3777. Doctors can download a digital referral form HERE
All clinicians at the Institute for Sports Physiotherapy and Performance are first-contact practitioners, meaning a referral is not required to book an appointment. Your private insurance provider may require a referral prior to the session for reimbursement. Please contact your insurance company if you have questions about your plan.
